Or, if you are ready to get started, please complete our New Client Application. The New Client Application forms for you and your doctor to complete may be found below. If you would please complete the Medical Statement and Liability Release and email back to us, and have your doctor complete the Doctor Consent Form and give he/she the letter requesting a Bone Density Scan, which is required for all clients who are more than 6 months post injury if they are not walking. The only exception is if they are under 18 years of age. There is also a form below “Welcome to NextStep Atlanta” with information for you on our programs and the process for participating.
We customize all of our programs to meet the individual’s needs and goals. Once we have received all required paperwork back from you and your doctor, we will schedule your evaluation which is $250 and is a 2 hour session. After your evaluation, our specialists will recommend the appropriate program for you: Guided Exercise, NMES (Neuromuscular Electrical Stimulation) or Gait Training, and will discuss Membership options.
